I don't see Rick, they all appear to have hands, if anything I think that the tower that she is in is at the Kingdom and they are beginning to move to The Saviors Base. But you maybe right about it being the First Battle.

Looking at it now and comparing it with next issue's cover (Issue 114) along with how things are going now...
It does appear that Holly, Nicholas, Heath, and Rick are down with The Saviors (see the four characters with no weapons) and they are maybe being marched to The Saviors Base as ransom or for execution or something. Perhaps they are possibly saved by Paul and his men who they intercept on the way to the Saviors Base.
